Job Description Job Description Position Overview The Licensed
Clinical Social Worker (LCSW) provides specialized psychosocial
support to patients with chronic, serious, and terminal illness and
their caregivers, with a primary focus on end-of-life
conversations, goals-of-care discussions, and family support
throughout the illness trajectory. This role works closely with an
interdisciplinary team to ensure patient care aligns with
individual values, promotes dignity, and supports both patients and
caregivers during advanced illness. The position requires strong
comfort with hospice and palliative care, complex family dynamics,
and emotionally sensitive clinical situations. This is a
field-based role requiring travel to patient homes. Key
Responsibilities Provide psychosocial assessment, counseling, and
emotional support to patients with chronic and terminal illness and
their caregivers Lead and facilitate end-of-life and goals-of-care
conversations, including discussions related to disease
progression, quality of life, and patient preferences Facilitate
and lead family meetings to support shared understanding,
decision-making, and care planning Provide grief counseling and
anticipatory bereavement support to patients and caregivers Serve
as a clinical resource for hospice and palliative care education,
including eligibility, philosophy of care, and care transitions
Support timely and appropriate hospice referrals when patient goals
of care align Collaborate with hospice agencies, community
partners, and internal care teams to ensure continuity of care
Identify psychosocial barriers and social determinants of health
impacting care and connect patients to appropriate resources
Collaborate closely with the interdisciplinary team, including
physicians, nurse practitioners, social workers, and community
health workers Participate in interdisciplinary team meetings, case
conferences, and care planning discussions Document all
assessments, interventions, and care coordination activities
accurately and timely in the medical record Travel to patient homes
and community locations as required Required Qualifications
Master’s degree in Social Work (MSW) from an accredited program
Active Licensed Clinical Social Worker (LCSW) license in the state
of Texas, in good standing Demonstrated experience or strong
comfort with serious illness, end-of-life conversations, and
emotionally complex clinical situations Reliable transportation and
ability to travel to patient homes Ability to work independently
while functioning effectively as part of an interdisciplinary team
Preferred Qualifications: Hospice and Palliative Care Certification
(or willingness to obtain) Prior hospice, palliative care, or
medical social work experience Experience providing grief
counseling and/or bereavement support Knowledge, Skills, Abilities
and Competencies Knowledge: Hospice and palliative care principles,
eligibility criteria, and care models ? Psychosocial impacts of
chronic, serious, and terminal illness Grief, loss, and bereavement
processes Community resources and supportive services relevant to
medically complex and aging populations Skills: Skilled
facilitation of sensitive conversations related to end of life and
goals of care Strong family meeting facilitation and conflict
navigation skills Excellent verbal, written, and interpersonal
communication Clinical assessment, care planning, and documentation
skills Abilities: Ability to manage emotionally intense situations
with professionalism, empathy, and resilience Ability to build
trust quickly with patients, caregivers, and care teams Ability to
prioritize and manage a caseload in a field-based environment
Ability to collaborate effectively within an interdisciplinary team
Competencies: Demonstrates compassion, cultural humility, and
patient-centered care Maintains professional boundaries while
providing meaningful emotional support Exercises sound clinical
judgment in complex and sensitive situations Adheres to ethical
standards, regulatory requirements, and organizational policies
Compensation & Benefits: Salary range: $75,000 - $92,000 annually,
depending on experience. Comprehensive benefits package including
