Job Description:

Licensed Vocational Nurse (LVN) to provide nursing care to adult
patients in their homes. This position offers a unique opportunity
to be an integral part of our committed team helping adult patients
to enjoy a higher quality of life in the comfort of their own
environment. The ideal candidate will have a passion for community
health and a strong commitment to patient care. Provide direct
nursing care according to the prescribed patient care plan. Monitor
patient health and record keeping observations and medical details.
Administer medications and treatments as prescribed. Collaborate
with physicians and multidisciplinary team members. Provide health
education to patients and their families. Ensure compliance with
all health and safety regulations. Strong clinical skills in a home
health setting. Excellent communication and interpersonal skills.
Proficient in using healthcare software and patient management
systems. Ability to work independently and manage time effectively.
Medical Insurance Dental Insurance Vision Insurance Life Insurance
Current LVN License in the state of practice. Valid driver’s
license and reliable transportation. Basic Life Support (BLS)
certification.
